Knee Osteoarthritis to Increase in Younger Age Groups in Next Ten Years

Based on current trends, nearly 6.5 million Americans between the ages of 35 and 84 are expected to be diagnosed with knee osteoarthritis in the next 10 years, according to research presented this week at the American College of Rheumatology Annual Scientific Meeting in Chicago. The study also reports that people as young as 45 to 64 years old will account for more than half of newly-diagnosed cases – increasing the need for total knee replacements and placing an additional burden on the United States health care system.

Senior author of the study, Elena Losina, PhD, PhD; co-director of the Orthopedics and Arthritis Center for Outcomes Research at the Brigham and Women’s Hospital in Boston and lead investigator of the study comments, “The large number of newly-diagnosed cases of knee osteoarthritis in younger individuals will lead to continued increases in the use of total knee replacement. Furthermore, these data are consistent with the recently observed tripling of total knee replacement use in 45 to 65 year old persons in the U.S.”

Osteoarthritis, or OA as it is commonly called, is the most common joint disease affecting middle-age and older people. It is characterized by progressive damage to the joint cartilage—the cushioning material at the end of long bones—and causes changes in the structures around the joint. These changes can include fluid accumulation, bony overgrowth, and loosening and weakness of muscles and tendons, all of which may limit movement and cause pain and swelling.

Researchers from the Brigham and Women’s Hospital used data from the Centers for Disease Control in a validated computer model (called the OAPol model) of the course of knee osteoarthritis to estimate the number of newly-diagnosed knee OA cases in the U.S. during two decades: the 1990’s and the 2010’s.

Using the OAPol model, the researchers were able to simulate the onset and progression of knee OA in individuals with demographic characteristics similar to U.S.-based population from age 25 until death. Data from the model were combined with CDC population estimates to project the incidence and course of knee OA in the U.S. during the time periods studied.

The researchers found that physician-diagnosed knee OA affects about seven percent of Americans over the age of 45.They estimate that Americans will be diagnosed with knee OA much earlier in life in the 2010s versus the 1990s.In fact, the average age of physician-diagnosed knee OA is projected to fall from age 72 in the 1990s to age 56 in the 2010s – a difference of 16 years. Additionally, they found that Americans who are between the ages of 35 and 84 in the early 2010s are likely to account for nearly 6.5 million new cases of knee OA over the next decade, with ages 45 to 64 accounting for 59 percent of these cases.

The results further revealed that Americans ages 45 – 54 at the beginning of the 1990s would account for an estimated 412,214 newly-diagnosed knee OA cases over the following 10 years. Among this same age group, the study found that in the 2010s, newly-diagnosed knee OA cases over the next 10 years are expected to exceed two million. That means about five percent of all Americans ages 45 to 54 would be diagnosed with knee OA over the next decade of their life – compared to only 1.5 percent, based on the data from 1990s.

“In the last decade, obesity and knee injuries have become more prevalent, possibly contributing to the increase in knee OA in younger adults,” explains Dr. Losina. “These trends toward earlier knee OA diagnosis may have a dramatic economic impact on the U.S. health care system.”

Background/Purpose: Symptomatic knee osteoarthritis (OA) affects about 7% of Americans over the age of 45. Obesity and knee injuries have become more prevalent in recent years, which may contribute to increasing incidence of symptomatic knee OA, particularly in younger age groups. Temporal trends in the age of symptomatic knee OA onset and the resulting public health impact have yet to be studied.

Method: We used the OAPol Model, a validated computer simulation model of the natural history of knee OA, to estimate the age-stratified 10-year incidence of symptomatic physician-diagnosed knee OA during two distinct time periods (the 1990s vs. the 2010s). Symptomatic knee OA incidence data for the 1990s were derived from published literature. Data for the 2010s were derived from the National Health Interview Survey 2007-08. We followed a simulated cohort from age 25 to death to track the onset of symptomatic knee OA. We coupled model output and CDC population estimates from each time period to project the number of incident cases of knee OA in the US population over each 10-year period. We compared ages of OA onset and 10-year cumulative incidence of knee OA for the two time periods.

Result: The mean age of physician-diagnosed knee OA onset fell from 72 ± 12 years (mean ± standard deviation) in the 1990s to 56 ± 18 years in the 2010s. Americans ages 35-84 in the early 2010s were expected to incur 6,475,642 incident cases of symptomatic knee OA over the next decade, with those ages 45-64 accounting for 59% of these cases (Figure). Among Americans who were ages 45-54 at the beginning of the 1990s, an estimated 412,214 incident cases of knee OA were expected over the subsequent 10 years, resulting in a 10-year cumulative incidence of 1.5%. Among people in the same baseline age group in the 2010s, 2,108,881 incident cases of symptomatic knee OA are expected over the next 10 years, a cumulative incidence of 4.8%. The projected number of 10-year incident cases among those ages 65-74 at baseline decreased slightly between the 1990s (1,169,615 cases) and the 2010s (882,997 cases).

Conclusion: Since the early 1990s, the age of onset of physician-diagnosed symptomatic knee OA has shifted dramatically, occurring on average 16 years earlier in life. This trend may reflect temporal changes in the prevalence of OA risk factors, as well as thresholds for patient care-seeking and physician diagnosis of OA. If the current OA incidence trend continues, nearly 6.5 million Americans between the ages of 35 and 84 will be diagnosed with symptomatic knee OA in the next 10 years, with those ages 45-64 accounting for more than half of these incident cases. A resulting spike in the utilization of healthcare, specifically total knee replacements, could have a dramatic economic impact and place additional burden on the healthcare system.

France’s RTL radio recently reported that Poly Implant Prothese (PIP) a now defunct French manufacturer of breast implants was using industrial grade silicone. Due to an unusually high rupture rate it was discovered the company was using industrial grade silicone, not medical grade for their implants. Industrial grade silicone includes; Baysilone, Silopren and Rhodorsil which were never approved or tested for clinical use, but used as a fuel additive and even in mattresses. A Dutch company “Rofil M-implants” rebranded the PIP implants and are also at risk as the contents are the same.

According to an article published by ABC News dated January 4, 2012, PIP was shut down in 2010 and the implants banned when the discovery was made. Prior to this the company grew to be the world’s third largest manufacturer of breast implants. The prices for PIP implants were unbeatable. According to Wikipedia, industrial grade silicone costs only 10% of approved medical grade.

Dr. Miguel Delgado advises any woman who had breast augmentation surgery with PIP silicone implants to contact her surgeon to discuss any concerns. Dr. Delgado stresses the importance for women to know what brand, what size and if their implants are silicone or saline.

The Food and Drug Administration had banned all silicone implants from 1992-2006 due to possible health issues but saline implants which never caused such concerns remained on the market. PIP sold a line of saline-filled implants starting in 1996 in the United States, and silicone implants outside the United States. In a recent report in HealthNews.com, in 2000 the FDA sent an inspector to PIP’s manufacturing plant in France who found multiple violations of accepted manufacturing practices and banned all PIP implants in the U.S.

Silicone breast implants were allowed back on the market in the United States in 2006 after being banned for 14 years. Extensive studies showed approved silicone implants are safe. Mentor, one of the two U.S. approved manufacturers has published a statement regarding their approval by multiple regulatory agencies verifying safety and health standards of their breast implants.

ASAPS (The American Society for Aesthetic Plastic Surgery) has published a guide for members with patients that have PIP silicone implants. ASAPS states that the PIP implants have a higher rupture rate than other available implants and since the adverse effect of the impurities used in industrial grade silicone is unknown, removal of the implants is recommended. ISAPS (International Society of Aesthetic Plastic Surgery) has come out with their own statement supporting this recommendation.

Many mixed reports are coming in from different countries with different recommendations. Some countries recommending removal besides the United States are: France, Venezuela, Germany and the Czech Republic. Israel has set up a hotline to deal with concerned patients.

The French government stated that the industrial grade silicone was more irritating to tissues if the implants rupture and therefore recommends having them removed. France is covering the cost of removing such implants.

The President of the Australian Society of Plastic Surgeons advises there is no need for alarm, but is encouraging all surgeons and patients to register all implant details, so should the need arise they can contact people in a timely manner.

Britain concluded there was not sufficient evidence to recommend routine removal, finding no link between the implants and cancer.

Bibliothèque nationale de France (BNF), the National Library of France has selected EBSCO Discovery Service™ (EDS) as its library discovery solution. As a long time EBSCO Publishing (EBSCO) customer, the National Library of France was pleased with EDS’s comprehensive index solution and ability to incorporate many of the library’s information resources. After looking at several discovery services, the librarians at BNF were confident that EDS was the best service to expose users to the national library’s collection — one of the largest and richest e-collection of any library worldwide.

BNF had been looking to modernize its aging information system and librarians were impressed with the impact EDS could have on the library — particularly the depth and breadth of coverage in the Base Index of EDS. Before looking at EDS, librarians had been unfamiliar with this type of comprehensive index. Demonstrations by EBSCO’s sales team made it clear that an index the size of EDS’s would be capable of pulling together content that might otherwise go undiscovered.

BNF Director of the Science and Technology section Hervé Colinmaire, says, “We were pleased with the index solution in EDS and feel that it will work well with our library. What interested us most in the EDS index solution is the capacity of the search engine to retrieve information at the smallest and deepest level of information.”

BNF’s information system consisted of many different layers of information without any integration. Colinmaire says EBSCO was the right fit to bring together BNF resources while providing easier access for the end user. “EDS provided our users the possibility to browse through a ‘jungle’ of resources dispatched in different tools and parts of our site. We think that our library will benefit greatly from EDS as we’re absolutely confident that the vast amount and range of our resources will be much better known and publicized.”

Colinmaire says other important factors in the decision to implement EDS as the discovery service at the National Library of France was the unique ability to incorporate French speaking resources into EDS as well as licensed and full text databases. “We believe EDS was the right fit for our library because it provides the best coverage rate possible for our collection and the best opportunity to grow with our collection. We were also particularly interested in the capacity of EDS to give access to our French resources allowing our patrons to access as much content as possible in one place.”

Future plans include the integration of EDS with the BNF OPAC to further expand the integrated one-stop search experience for users. EDS customers build upon the base index by adding metadata representing their own unique collections including their catalog (OPAC), databases, archival collections—using EDS as the base of an institution-specific discovery experience.

BNF also plans to also taking advantage of a number of the customizability options available within EDS to include value-added features to further create a unique discovery solution for the National Library of France. BNF is currently planning to create LibGuide widgets on the result list and to customize the EDS interface colors.

EBSCO Discovery Service creates a unified, customized index of an institution’s information resources, and an easy, yet powerful means of accessing all of that content from a single search box-searching made even more powerful because of the quality of metadata and depth and breadth of coverage.

Three local musicians, all actively involved in community projects, have set up a new after-school project to teach underprivileged children a musical instrument. The program focuses on kids between the ages of 11 – 16 years who come from families that could not otherwise afford music lessons or the price of renting or buying a musical instrument. The idea is to encourage kids who wouldn’t otherwise have the opportunity, to get involved with music. It also gives exposure to classical music and the chance to learn from classically trained musicians. Based in a local community center, the project runs Monday through Thursday in the afternoons.

“In addition to weekly lessons, we provide a welcoming and supportive environment for the kids to come and practice after school as well as free lessons, we also provide the instruments,” comments Judith Leist, one of the three musicians who started the project. The price of renting or buying musical instruments is said to be one of the biggest obstacles for children from poorer families in starting to learn an instrument.

The three musicians who started the project – Beth Stollman, Judith Leist and Dennis Flint are all actively involved on a daily basis in giving the lessons. Students have the opportunity to study violin, cello, flute or clarinet. “Depending on resources, we hope to expand that to also include singing lessons,” comments Judith Leist. They are supported by a host of volunteers and have received instruments donated from people throughout the area.

One of the initiators of the project, Judith Leist, explains; “Music is such a positive and constructive outlet. Not only does it provide a constructive way to spend spare time, it also allows children to express themselves. Additionally, studying music also helps children with concentration and even mathematics.”

So far more than 40 children are learning to play an instrument in the program. They are aiming to put together a spring concert in March next year for family and those who have generously donated to the project.

Judith Leist is a keen musician and has played in professional orchestras in Europe as well as in the U.S. She has been actively involved with many charities and community projects over the years and donates a considerable amount of her time to performing for charity events.
